FOUNDATIONAL COHORT PARTICIPATION AGREEMENT
AND CONFIDENTIALITY ACKNOWLEDGMENT
BY PARTICIPATING IN THIS EVENT, YOU (HEREIN REFERRED TO AS “PARTICIPANT”) AGREE TO THE FOLLOWING TERMS STATED HEREIN.
This Foundational Cohort Participation Agreement (the “Agreement”) is between FysioCanix, a Walker Chiropractic Corporation, a California corporation with its principal place of business at 11704 Red Hill Court, Gold River, CA 95670 (hereinafter, the “Company,” “we,” “us,” “our”) and:

- Program/Event
The Company agrees to provide Participant access to the FysioCanix Foundational Cohort (the “Event”), including Course Classes (video, audio, course materials, etc., via the secure website portal) and a one-day, in-person, no-cost educational and feedback session held on August 22, 2026. Participant agrees to abide by all policies and procedures outlined in this Agreement as a condition of their participation in the Event.
- No Cost; No Certification
The Event is provided to Participant at no charge ($0.00). Participation in the Event does not constitute enrollment in, and is not a substitute for, the FysioCanix Certification Program. No certification, credential, designation (including but not limited to “Certified FysioCanix Chiropractic Practitioner” or “CFCP”), or continuing education credit is earned, implied, or conferred through participation in the Event. Participant is not authorized to represent to any third party that they hold FysioCanix certification as a result of attending the Event.
- Purpose of the Event
The Event is intended to:
- Introduce Participant to the SETA Method (See Everything to Treat Anything) and related FysioCanix curriculum concepts;
- Gather structured feedback from Participant regarding course content, delivery, and materials; and
- Provide Participant the opportunity to evaluate whether the full FysioCanix Certification Program is of interest and if their enrollment would benefit both the Participant and our Profession.
The Company makes no representation that Participant will be offered enrollment in, or any particular pricing for, the full Certification Program as a result of attending the Event.
- Confidentiality
Participant acknowledges that during the Event they will have access to certain confidential and proprietary information of the Company (“Confidential Information”), including but not limited to the SETA Method, the 9-Step Roadmap to Wellness, course materials (including forms), proprietary presentations, audio and video content, training methodologies, and any other information disclosed by the Company that would reasonably be understood to be confidential given the nature of the information and circumstances of disclosure.
Participant agrees to:
- Maintain all Confidential Information in strict confidence and not disclose or use such information for any purpose other than Participant's own professional development;
- Not reproduce, record, photograph, or distribute Confidential Information without the Company's prior written consent; and
- Use at least the same degree of care to protect the Confidential Information as Participant uses to protect their own confidential information, but in no event less than reasonable care.
This confidentiality obligation shall survive the conclusion of the Event indefinitely.
- Non-Disclosure of Materials
Materials provided to Participant in connection with the Event are proprietary, copyrighted, and developed specifically for the Company. Such materials are for Participant's own personal, professional use only. Any disclosure, reproduction, or distribution to a third party is strictly prohibited. No license to sell, distribute, or use Company materials for Participant's own business or training purposes is granted or implied.
- Feedback and Contribution License
The Company will invite Participant to share feedback, comments, testimonials, or other content (“Contribution”) during or after the Event. Participant agrees that the Company may use, reproduce, and reference Participant's Contribution for internal program development and, with Participant's separate consent, for marketing or promotional purposes. Participant represents that any Contribution they provide does not contain confidential patient information, protected health information, or any material that violates applicable privacy laws, including HIPAA.
- Assumption of Risk; Limitation of Liability
Participant agrees they attend the Event at their own risk and that the Event is an educational and informational session only. Participant releases the Company, its officers, employees, directors, and related entities from any and all claims or damages arising from Participant's attendance at or participation in the Event, to the fullest extent permitted by law.
IN NO EVENT SHALL THE COMPANY'S LIABILITY ARISING OUT OF OR IN CONNECTION WITH THIS AGREEMENT OR THE EVENT EXCEED ANY AMOUNTS PAID BY PARTICIPANT TO COMPANY FOR THE EVENT (WHICH THE PARTIES ACKNOWLEDGE IS $0.00).
- Professional Conduct
Participant agrees to conduct themselves professionally throughout the Event. The Company reserves the right, at its sole discretion, to limit or end Participant's participation in the Event if Participant becomes disruptive or non-compliant with Event policies.
- No Guarantee of Future Terms
Participant understands that any pricing, curriculum structure, or enrollment terms discussed or referenced during the Event for the full FysioCanix Certification Program are subject to change and do not constitute an offer. Any future enrollment in the Certification Program will be governed by a separate Enrollment Agreement executed at that time.
- Counterparts; Signatures
This Agreement may be executed in one or more counterparts, including by electronic signature, each of which shall be deemed an original, and all of which together shall constitute one and the same agreement.
- Governing Law
This Agreement shall be governed by and construed in accordance with the laws of the State of California, without regard to its conflict of laws provisions.
- Entire Agreement
This Agreement constitutes the entire agreement between the parties with respect to the Event and supersedes all prior discussions or understandings relating to the Event. This Agreement may not be modified except in a writing signed by both parties.
- Notice of Non-Attendance
If Participant becomes unable to attend the August 22, 2026 event, Participant agrees to notify the Company as soon as reasonably possible so that the seat may be offered to another doctor on the waiting list. This Agreement does not involve any monetary payment and, as such, no refund or cancellation fee provisions apply.
